FORM HAS
TYPE (SELECT - 2 products // Individual // Family)
AGE (SELECT - 6 options // <22 // 22-29 // 29-39...)
COVER (SELECT - 5 options // $1000 // $1500 // $2000...)
PREMIUM (text box / div area - displays PREMIUM per month based on selects)
then - would you like to add a family member (extended)
YES // NO
if yes (3rd product will only be available if FAMILY is selected)
AGE (SELECT - 6 options // <29 // 29-39 // 39-49...)
COVER ( SELECT - 5 options // R10000 // R15000 // R20000...)
Possible to add 2 members
AGE (SELECT - 6 options // <29 // 29-39 // 39-49...)
COVER ( SELECT - 4 options // R10000 // R15000 // R20000...)
then ADD premium of prod 2 (family) to prod 3 (family extended)
I have a CSV with all the values hard-coded (no calculations) based on the fields selected.

all i want is to
select individual, 39-49, R10000 and it must show R(51)p/m
OR
select family, 29-39, R15000 and it must show R(86)p/m
AND if i want to add family (EXT)
select <22, R25000 and it must show R(26)p/m
then ADD the family amounts... 86+26 = R(112)p/m
